Registration Info

In order for us to accept a camper’s registration, we must receive the following:

Complete Application
Emergency Form
Copy of your child’s Physical (dated within 24 months of camp attending)
Copy of your child’s Immunization record
Full payment

Confirmation Letter

Once we have received your complete registration packet, a letter will be sent confirming your enrollment in Lazer Lax Lacrosse Camp. This letter will include information regarding what your son will need for the camp.

Waiting List

If we receive a registration that is incomplete, you will receive notification from Lazer Lax indicating what is missing. Your son will be placed on the waiting list until the missing item(s) has/have been received.

Cancellation Policy

All cancellations must be made prior to one week in advance of the start of the camp. Any cancellations made prior to one week in advance of the start of camp will be refunded the enrollment cost less a non-refundable registration fee ($50 clinic, $100 camp). There will be no refund for any cancellation made within one week of the start date of camp or for any missed days during the week of camp.

Insurance

Every camper must be covered by family or other accident and health insurance. Parents are encouraged to consult with their insurance advisor to ensure that campers have adequate coverage.

Health Forms

The Massachusetts Department of Health and the local Andover Board of Health require each camper to have a physical exam and a complete immunization record. In order to meet these regulations, a copy of both must be submitted with your child’s application.
